A realistic guide for patients
The neck is often the first area to show signs of aging: laxity, loss of contour, beginning band formation or a less defined jaw-neck line. The question of whether a necklift without a full facelift is possible therefore frequently arises during consultations.
The short answer? Yes, an isolated necklift is perfectly possible, but only if the anatomy lends itself to it. What is and is not possible depends entirely on the distribution of volume, the quality of the skin and the degree of laxity in the face itself.
When is a necklift without facelift suitable?
1. When the sagging volumes in the face are limited
A necklift works excellently when the problem is primarily in the neck and the overlying face still has sufficient support and projection. In other words:
- The cheeks are not sagging
- The jawline is still relatively sharp
- The midface has not descended
In this situation, an isolated necklift can restore the contour without requiring extensive treatment of the face itself.
2. In younger patients who desire a “different” neck
An isolated necklift occurs remarkably often in younger patients who have always had a genetically heavier neck. Not due to aging, but due to genetic factors:
- A shorter neck
- Fuller submental fat
- A less defined jaw-neck angle
- A naturally heavier profile despite young age
In this case, the procedure borders more on transformative surgery than rejuvenation surgery.
The goal is not to return to a previous version of oneself, but to achieve a slimmer, longer and more defined neckline, something they never naturally had.
This group often responds exceptionally well, because the skin quality is still excellent and the anatomy is easily correctable.
When is a necklift without facelift not suitable?
1. When the entire face has aged significantly
A necklift alone cannot provide a solution for:
- Forward-hanging cheeks
- Jowls
- A fading jawline
- Volume loss in the midface
If these zones show the same degree of laxity as the neck, a necklift without facelift creates imbalance: a young neck under an older face. In that case, a combined Deep Plane Facelift + deep necklift is the most harmonious option.
2. When skin excess and volumes of the face influence the sagging in the neck
Many patients think their problem is “only the neck,” while the laxity actually comes from above. This often becomes apparent when the neck moves along as the patient pulls up the cheeks with their fingers. When cheek tissue descends, it directly impacts the neck; optimal neck contouring can only be achieved if the mid and lower face are lifted concurrently. Leaving the mid or lower face untreated may lead to a premature relapse of neck laxity.
The anatomical reality of the neck: what can surgery solve?
A necklift can effectively correct:
- Excess skin under the chin
- Loose platysma muscles (band formation)
- Superficial and deep fat
- A poor cervicomental angle
- A short or full neck
It cannot correct:
- Sagging cheeks
- Jowls
- Descent of the midface
- A poor jawline due to facial sagging
For those issues, the face itself is the cause, not the neck.
So, when is an isolated necklift sufficient?
Suitable when:
- The neck is the only real problem area
- The face still looks firm and relatively young
- There is genetic fullness in the neck in younger patients
- The wish is more contour improvement than rejuvenation
Not suitable when:
- Facial and neck aging go together
- The jawline and cheeks are sagging
- The midface is lax
A necklift without facelift is a beautiful procedure, for the right patient. It works excellently for younger patients with genetic heaviness of the neck and for people whose face is still firm but whose neck is clearly aging faster.
When aging is more broadly distributed, however, a combined Deep Plane Facelift provides the most natural and harmonious rejuvenation.
