Upper eyelid surgery is usually performed in an outpatient setting, unless it is part of a combined rejuvenation treatment. The procedure typically takes approximately 45 minutes.

During the procedure, the excess skin tissue is carefully marked, so that the scar after the procedure falls neatly within the natural crease of the upper eyelid.

In most cases, it involves the removal of excess skin, but if necessary, fat tissue can also be removed or repositioned where needed.

Lower eyelid surgery

Lower eyelid surgery focuses primarily on reducing puffiness and skin wrinkles.

Dr. Van Genechten always applies a technique in which the orbicularis muscle around the eye is not incised, which means that there are typically no visible scars.

This technique provides a subtle result without the signs of a surgical procedure. In some cases, the skin texture or limited excess skin is corrected with a laser treatment or a minimal skin excision.

In addition, fat compartments under the eyes are repositioned or removed to create a fresh and youthful appearance. If necessary, the transition between the eyelids and the upper fat compartments of the cheek is refined with fat transfer, using fat from the abdomen or thigh.

Combination of upper and lower eyelid surgery

Sometimes upper and lower eyelid surgery are performed at the same time, depending on your specific wishes and facial features.

An upper eyelid surgery is also frequently combined with a brow recontouring or brow reset.

Recovery

Following upper or lower blepharoplasty, there may be significant swelling and discoloration, especially if fillers have previously been applied in this area. The sutures are usually removed after 7 days, after which you will notice rapid improvement. During the first 1 to 2 weeks, swelling and discoloration will gradually subside, and most patients can resume their daily activities. Within 2 months, the final result will be visible once the skin has fully healed.

The goal of eyelid surgery is to give the skin around the eyes a younger, tighter appearance, to improve volume distribution and to resolve any vision impairment caused by drooping eyelids. The result is a fresh and bright look that makes a difference both aesthetically and functionally.

What are the risks of eyelid surgery?

Eyelid surgery is a surgical procedure, and as with any operation, there are always certain risks and potential complications. Although most patients do not experience problems, it is important to be fully informed about the benefits, risks and limitations of the procedure.

Does eyelid surgery hurt?

A facelift is ultimately a trade-off in anatomy and vectors. Aging creates excess and laxity. Rejuvenation requires repositioning that tissue somewhere.  The question is not whether a trace exists. The question is where you place it, how much tension it carries, and whether natural contours are respected.  These scars are approximately 6 weeks after surgery. They will continue to soften and mature over the coming months.  But scar quality is rarely just about “healing.”
It is heavily influenced by tension, vascularity, smoking status, incision design, and how anatomy is respected during closure.  Around the ear, anatomy becomes particularly unforgiving. Small distortions immediately reveal surgery:
- loss of the earlobe contour
- displacement of the hairline
- flattening of natural transitions
- visible pull in front of the ear  That is why I generally prefer hiding the incision within natural reliefs and contours whenever possible, including partially inside the tragal border, rather than accepting visible distortion elsewhere.
